Diagnosis
Bone marrow aspiration is mandatory for all patients in whom CML is suspected, as it will confirm the diagnosis (e.g., cytogenetic analysis), and provide information needed for staging in terms of the blast and basophil percentages. Baseline reverse transcriptase-polymerase chain reaction is imperative to identify the specific type of rearrangement that can be appropriately followed when assessing for response to TKI therapy.

Danach ergeben sich aus der Vielzahl der Optionen die Chancen auf die gezielte, richtige Auswahl der Therapieoptionen. Derer gibt es ja mittlerweile viele, wie auch in diesem Artikel dargelegt. Leider in englischer Sprache.

CAMBRIDGE, Massachusetts, und LAUSANNE, Schweiz--(BUSINESS WIRE)--ARIAD Pharmaceuticals, Inc. (NASDAQ:ARIA) gab heute den Start einer randomisierten Phase-3-Studie zu Iclusig® (Ponatinib) als Zweitlinientherapie bei Patienten mit chronischer myeloischer Leukämie (CML) in der chronischen Phase (CP) bekannt. Die Studie OPTIC-2L (Optimizing Ponatinib Treatment In CML, Second Line) vergleicht die Wirksamkeit und Sicherheit von Ponatinib in zwei Studienarmen mit verschiedenen Startdosierungen mit Nilotinib bei Patienten mit Resistenz gegen die Erstlinientherapie mit Imatinib. Der primäre Endpunkt der OPTIC-2L-Studie ist das gute molekulares Ansprechen (MMR; Major Molecular Response) nach 12 Monaten. Die Patientenrekrutierung wurde nun gestartet, und voraussichtlich werden etwa 600 Patienten in klinischen Prüfzentren in Europa, Asien, Lateinamerika und Kanada in die Studie aufgenommen werden.

Wichtige Aspekte des Designs der Studie

In diese für den Nachweis der Überlegenheit von Ponatinib gegenüber Nilotinib ausgelegte Studie werden Patienten mit CP-CML aufgenommen, die eine Imatinib-Therapieresistenz entwickelt haben und mit keinem anderen Tyrosinkinase-Inhibitor behandelt wurden. Diese Patienten erhalten randomisiert entweder Ponatinib in einer Anfangsdosis von 30 mg (Kohorte A) oder 15 mg (Kohorte B) einmal täglich, oder 400 mg Nilotinib zweimal täglich (Kohorte C). Die Randomisierung der Patienten erfolgt dabei in einem Verhältnis von 1:2:1. Bei Erreichen eines guten molekularen Ansprechens wird bei Patienten in Kohorte A die Tagesdosis von Ponatinib auf 15 mg und bei Patienten in Kohorte B auf 10 mg reduziert.

Der primäre Endpunkt der Studie ist das gute molekulare Ansprechen nach 12 Monaten in den einzelnen Kohorten. Zu den sekundären Endpunkten gehören die Rate der Gefäßverschlussereignisse in jeder Kohorte, die Nebenwirkungsrate und die Rate schwerer Nebenwirkungen.

(NASDAQ:ARIA) today announced the initiation of a randomized Phase 3 trial of Iclusig® (ponatinib) in second-line patients with chronic myeloid leukemia (CML) in the chronic phase (CP). The OPTIC-2L (Optimizing Ponatinib Treatment In CML, Second Line) trial is designed to investigate the efficacy and safety of ponatinib, administered at two starting doses, compared with nilotinib, in patients who are resistant to front-line treatment with imatinib. The primary endpoint of the OPTIC-2L study, now open for patient enrollment, is major molecular response (MMR) by 12 months. Approximately 600 patients are expected to be enrolled at clinical sites in Europe, Asia, Latin America and Canada.

“Physicians treating CML patients will be extremely interested in the outcome of this clinical trial in which ponatinib -- at two different doses -- will be compared to nilotinib in patients resistant to imatinib,” stated Dr. D. Major Design Features of the Trial

This study is designed to demonstrate superiority of ponatinib over nilotinib and will enroll patients with CP-CML who have become resistant to imatinib and have received no other tyrosine kinase inhibitor. These patients will be randomized to receive once-daily administration of ponatinib at a starting dose of either 30 mg (cohort A) or 15 mg (cohort B), or 400 mg of nilotinib administered twice daily (cohort C). Patients will be randomized in a ratio of 1:2:1 respectively. Upon reaching MMR, patients in cohort A will have their daily dose of ponatinib reduced to 15 mg and patients in cohort B will have their dose reduced to 10 mg.

The primary endpoint of the trial is MMR by 12 months for each cohort. Secondary endpoints include rate of vascular occlusive events in each cohort, rates of adverse events and rates of serious adverse events.

“The OPTIC-2L trial is the first direct randomized comparison of ponatinib to an approved BCR-ABL tyrosine kinase inhibitor following imatinib therapy. We expect this trial to provide important head-to-head data regarding the efficacy and safety of treating patients with ponatinib versus nilotinib in the second-line,” said Frank G. Renal artery stenosis has occurred in some Iclusig-treated patients and can present with worsening, labile or treatment resistant hypertension. In the event of significant worsening, labile or treatment-resistant hypertension, interrupt Iclusig treatment and consider evaluating for renal artery stenosis.

Abstract
The efficacy of second-line treatment for Chronic Myeloid Leukemia (CML) plays an important role in allowing CML patients to enjoy a normal life expectancy.
Four Tyrosine Kinase Inhibitors (TKIs) are presently available: bosutinib, dasatinib, nilotinib, ponatinib.
Each one has different safety and activity profiles, which are reviewed here.

No controlled studies are available to guide treatment decision, which must be based on the characterization of leukemic cells, especially in cases of resistance to TKI, coupled with the safety profile of each TKI.
Patient comorbidities also play an important role in the treatment decision, which can achieve a new durable response in over 50% of treated patients.

Compound mutations in BCR-ABL1 are not major drivers of primary or secondary resistance to ponatinib in CP-CML patients

Key Points

Ponatinib induces durable responses regardless of baseline BCR-ABL1 mutation status in CP-CML patients.

No single or compound mutant consistently confers primary or secondary resistance to ponatinib in CP-CML.

Durable cytogenetic and molecular responses were observed irrespective of baseline mutation status, and included patients with compound mutations. No single or compound mutation was identified that consistently conferred primary and/or secondary resistance to ponatinib in CP-CML patients. Ponatinib is effective in CP-CML irrespective of baseline mutation status.

Conclusions:
Real-world US data shows ponatinib is prescribed across disease phase, therapy line, and mutation status.
While a majority of patients were in their 3rd line of therapy or later, a substantial proportion of patients, especially in AP CML and Ph+ ALL, received ponatinib as 2nd line therapy.
Physicians appear to be selecting patients who are younger than those enrolled in registrational trial for ponatinib (55 years vs. PACE trial median age, 64 years),and mitigating against potential risk using lower starting doses and dose reduction.
Most prescribers have only 1 ponatinib patient, but physicians with >1 ponatinib patient favor lower starting doses.
Dose reduction and discontinuation occurred steadily over time rather than clustered at routine response milestone time points.
CP CML Patients starting at 15 mg appear to have similar or better treatment duration compared with those started at higher doses.

Ponatinib-associated hypertension was usually reversible after DC and most vascular events with subsequent TKIs occurred in patients with prior such events while on ponatinib.

Conclusion:
The combination of hyperCVAD with ponatinib is highly effective in pts with Ph+ ALL.
Due to the vascular events observed, the ponatinib dose was modified to 30 mg daily during consolidation with subsequent reduction to 15 mg in pts in CMR without further episode of cardiovascular events.
